Explanations

Copy of your driver’s license or state-issued ID.
This is used as proof of who you are, and to assure providers you’re able to drive to assignments you’ve chosen. You’ll need to include both the front and back, so you can easily upload images right from your smartphone’s camera.

Copy of your state-issued healthcare license.
This is your state-issued license. If you don’t have a copy of the license handy, you can simply upload your license number, state it was issued in and expiration date – we can check the rest. If you have multiple licenses, you’ll need to upload the current license appropriate for the assignment levels you choose. For example, if you have a C.N.A. certificate and an L.P.N. license, you can only apply for L.P.N.-related assignments if you’ve uploaded your valid L.P.N. license information.

Background check consent.
For your safety, and the safety of the patients you’ll care for, we will need your consent to conduct a background check. Checks may include: criminal history, illegal drug test consent, motor vehicle violations, Office of Inspector General (OIG) exclusions, and Adult Protective Services (APS) check-in states where required. The time it takes to complete a background check will vary from region to region. Upon completion of your application, you’ll be contacted by email with your approval OR get a notice that there may be additional information required in order to complete your application.

Personal photo.
Your photo identifies you as the person who accepted the assignment. So, whether you’re working in a facility, or visiting a private residence, you can demonstrate you’re the assigned caregiver through this photo. The photo can be taken and uploaded directly from your smartphone. After you’ve completed five (5) assignments through the GapNurse platform, you will receive an identification card that indicates you are a licensed GapNurse contractor. In addition, you’ll receive a downloadable link that stores this information right on your smartphone.

HIPAA_HITECH Business Associate Agreement.
As a licensed clinician, you’re probably very familiar with patient privacy laws and are already familiar with HIPAA. Healthcare providers that are paid through government insurances require Business Associate Agreements for their contractors. When you sign this form, you’ll be agreeing to protect and never share patient information you receive for assignments you accept.

Payment options.
GapNurse offers two different payment options: Weekly Direct Deposit or Same Day Pay

  1. The most common payment method is direct deposit. This is the “traditional” method of payment where you provide your bank account number, your routing number, etc. and payments can be paid in batches weekly. While bank rules vary, we have no control over how long it will take for your funds to clear your bank nor do we have control of any charges your bank might assess for these transactions. After the healthcare facility’s supervisor has reviewed your shift documentation, she/he click’s “approved”, and a payment is issued to the payment account you provided during registration. Depending on your bank procedures, payments normally arrive 3-5 days later. Please check with your bank before choosing this method of payment.
  2. Another option that you may be eligible to select is the “Same-Day-Pay” option. Same day pay is not immediately available for all accounts and may require a deposit history to establish. Once your assignment is finished, the GapNurse platform notifies the Case Manager/Assignment Supervisor of your completed work. Any notes you’ve provided through the platform will be instantly uploaded to that Supervisor. She/he then has the opportunity to confirm the assignment completion and approve the assignment for immediate pay. With the Same-Day-Pay option, money is immediately wired to your bank card on file. You will need to provide a debit or credit card for this to happen, but don’t worry, your information is securely encrypted and never stored in the GapNurse or Facility Manager’s files. We utilize a 3rd party card processor called Stripe (www.stripe.com) to guarantee your personal security. There is a small fee for these services, charged by Stripe. The fees normally range between $0.50 to 1.5% of the total amount paid. See Stripe.com for current charges.

Infection control records.
For many assignments, you will be required to provide vaccination records. While state requirements vary, the most common records include TB Test Results, Flu Vaccination (seasonal), and Hepatitis-B Vaccination/Waiver. Some opportunities may require these records in order to accept you. You can easily obtain these records from your current employer, or we can make arrangements on your behalf to receive them; just let us know.

Online interview.
The last step in the process is to participate in an online interview. Once your application file has been completed, you will be contacted by an interviewer to schedule a brief meeting at your convenience. This process is fun, easy, and typically takes less than 15 minutes to complete. During this process, we ask a few standard questions s and verify your identification. It can be conducted wherever and whenever you have an internet connection.
